were worst for prevalence of diabetes among adults among those 17 countries. worst for obesity across all age groups of those 17 countries. and had the worst infant mortality, the worst infant mortality of all 17 countries. we suffer higher death rates and worse outcomes for conditions such as heart disease and chronic lung disease. this chart from that institute of medicine report shows all these dots of the other countries grouped around cost, expenditure per capita and life expectancy. that's the united states of america, the dot with the red circle around it. we are an outlier, below virtually all of these countries except poland and turkey and hungary, below them all on life expectancy. they are all above 78 and we're just below it. and wildly out of the grouping on cost. we are way higher cost than the grouping of all of our industrialized competitors. we are wild outliers in a very bad direction of high costs and poor outcomes. this is a stark and unsettling disparity of us from virtually all the other nations, and it's not to our benefit. so the real issue here, the fact that

are based on their own participation in the system. an example of that is waiving co-pays for diabetics who take their medications. so those have a lot of momentum and i think you will see a lot and some of them are controversial. again i'm not going to take that on today because i want to get to two points which is employers are also very interested in the delivery system. what i have talked about so far as the employer benefit design by the employee. high deductible plans in value-based insurance but we obviously understand that no one is ever going to be healthy enough to not need to go to the doctors hospitals and drugstore so what happens in the delivery system is important. the same principles apply about those market forces. so there's a big move to show the kind of crisis that you see which happens to be in schenectady that actually was derived believe it or not by a group calling doctors offices and calling different kind of providers of the services to see this gigantic variation in price for something as simple as a sore throat. i showed this because we know that we are not going

that goes out to the farms. we have a 50-year-old farmer who has a history in his family of diabetes. he comes in and does not know that they can get care at a community health care. a lot of it is out of reach and enrollment into understanding both the cultural issues as well as being able to get them into care. >> let me switch gears and touch upon another very important issue in terms of how we determine reimbursement rates for physicians. that is the rock, which is not a household word through america, an organization that plays an important role in determining how specialists -- how much primary-care physicians will make. we have an organization that is top heavy with people in specialties. is this an important issue? >> when we talk globally about the idea of inequity, how do we get a handle on it? medicare is the largest single care and historically many cares key off medicare, managing the conundrum around the pay gap would be central to reforming the whole system. it is well wired in the sense that it has been this way for a long time. there are a variety of approaches to it. yo

. often from treatable causes like diabetes and smoking related chronic conditions. these consequences also spill into other areas. as any teacher or school counselor will tell you, a child who struggle with depression, anxiety, any other mental health condition, is also likely to struggle academically. it's also an issue for our justice system. since our -- we should be receiving mental health and substance abuse counseling instead. i have had a number of sheriffs in my own state, as well as other states, tell me that their jails, the jails are now the de facto mental institution in their states. the shame and this is that with access to the right treatment to support most people with mental goes can recover and lead productive and healthy lives. that we need to make the critical investments that will enable us to happen. so wearing my other hat as the chairman of the labor, health and human services appropriations subcommittee, i plan to take a close look at funding opportunities in this area through the appropriations process. we've made important steps forward in recent years. my f
